Leone's (Stallone) enemy Chink Webber drops a barbell on his friend and kills him. So Leone puts a weight in a pillow case and starts swinging it around like a maniac, and goes after Chink. This is the first time after about an hour and a half into the movie, stallone actually fights back. He gets his ass kicked the whole movie.
